Credentialing checklist for your MSO
These are the items we assemble and submit for each psychiatrist. Sharing your application packet early is the single biggest factor in shortening the timeline.
State medical license (and IMLC letter of qualification where applicable)
DEA registration for the state of practice
ABPN board certification or eligibility documentation
NPDB self-query, dated within 60 days
Complete work history with gap explanations
Three peer references, clinically relevant
Certificate of insurance and claims history
Immunisation, TB and fit-test records
BLS/ACLS and facility-required trainings
Photo ID, NPI and CAQH profile access
